Client Focused Case Management

Youth Case Management can assist young people who are dealing with difficult issues. Our youth workers can provide case management support, information and advice, supported referral and coordinated care.

Maintaining open lines of communication and engaging youth in their case planning provides them with the opportunity to be directly involved in decisions that affect and concern their lives. Youth participation in the decision-making process can be an effective approach in promoting self-esteem, a greater sense of control, and the acquisition of new skills.

To engage our Youth Case Management services complete the Case Management Referral Form and return in person or via email to youthvenue@singleton.nsw.gov.au.
